Customer Claims
Dear Customer,
This is in response to your request to file a claim. We make every effort to settle all claims within 120 days. The following instructions will help us to complete our investigation without any unnecessary delays:
Do not throw away any of the items for which you are submitting a claim. If the items were packed for shipment, retain the containers. Leave the items in the containers for our inspection.
Do not repair any items for which you are submitting a claim. We reserve the right to inspect all damaged items claimed prior to authorizing repairs to be done. Any repairs performed prior to inspection will invalidate your claim of the items.
Identify the items claimed by inventory number and by name. Describe the damage that you are claiming and complete in detail the following columns: Estimated weight, original cost, date acquired and amount claimed.
If your claim includes items that you are reporting as missing, please complete the Statement of Claim for such items just as you would for damaged items. On any item(s) you report as missing, it will be necessary for you to support ownership and value with a bill of sale, canceled check or some other evidence of a similar nature. In instances where a container is claimed missing, the contents of each such container should be listed and information submitted as to weight, date acquired, value before move and amount claimed. There may not be room for such listings on the Statement of Claim; supplemental sheets of paper are permissible, providing the information is listed as if it were on the regular claim form. List all items for a container together and designate the inventory number and the type of container. This will greatly assist us in our efforts to locate your property.
Keep your copies of the Bill Of Lading and Inventories from your move for possible inspection by our representative. The availability of these shipping papers will facilitate the handling of your claim. In Addition, it may be necessary for you to provide evidence of all charges on your move that have been paid. We cannot process a claim until your final charges have been paid in full.
